4118.2 Database Use Agreement

The Board of Education supports the use of data and believes access to databases will assist teachers in populating and utilizing data to ultimately increase learning.

Users of district database systems are responsible for their appropriate use. The databases placed on the school Filemaker server contain confidential information. Therefore, employees must maintain confidentiality and ethical use of this information and networks when they are granted access privilege.

In order to keep district database systems secure, users may not leave the terminal "signed on" when unattended and may not leave their password available in an obvious place near the terminal or share their password with anyone. The use and sharing of access information or database content is strictly prohibited.

Principals and supervisors shall be charged with the responsibility of reviewing security procedures and issuing I.P. numbers and passwords to teachers for access to school databases.

The principal/supervisor will reiterate the importance of maintaining database security. Once the employee understands the importance of database security and signs the Database Use Agreement, the appropriate I.P. number and password will be issued. The signed agreements will be forwarded to the district network administrator.

This access code will not be distributed to students, different site personnel or employee family members. Unauthorized use of this code will result in disciplinary action. The principal and staff member must sign Database Use Agreement.

I understand and will abide by the above Agreement. I further understand that a violation of the regulations above is unethical and may constitute a criminal offense. Should I commit any violation, my access privileges may be revoked, school disciplinary action and/or appropriate legal action may be taken.

Your signature on the Database Use Agreement is legally binding and indicates that you have received Regulation 4118.2, Database Use Agreement and that you have read the terms and conditions of this agreement.
