4321.1 Teacher Observation & Evaluation Procedures
PROCEDURES FOR PROBATIONARY TEACHER
Probationary Status: Moffat County Schools views probationary status for employees as part of the hiring process. The three years that an individual has in the probationary period are part of the growth and induction process into the school system. All probationary staff members will receive at least two (2) documented observations and one evaluation that results in a written summary report, each academic year.
First-Year Probationary Teachers: First year probationary staff members will be engaged in the appropriate induction/mentor program as determined by the building principal.
- Prior to September 15: Administrator and teacher will have an initial professional dialogue conference using the Professional Guide for Teaching as a framework for their discussion. The purpose of this conference will be to clarify the professional growth and evaluation procedures and teacher expectations. The conference will be documented and the teachers job description will be signed and forwarded to the personnel office.
- Prior to November 15: Administrator will conduct two (2) documented observations. These observations should be followed up with feedback from the supervisor within two school days after the observation. Form D - Documented Observation
- The teacher will conduct a performance Self-assessment using the Professional Guide for Teaching prior to second professional dialogue conference. Form A - Professional Guide for Teaching
- Prior to December 15: Administrator and teacher will have a second professional dialogue conference to identify strengths, areas for growth, and recommendations. - Form B - Professional Dialogue & Development Plan. Targets for goal development, and a timeline for continuous goal progress monitoring will be developed using Form C - Teacher Goals
- The administrator will conduct additional documented observations as determined by supervisor & teacher related to established goals
- Prior to March 1: Administrator and teacher will have an evaluation summary conference at which time goal progress will be evaluated, strengths and weakness and recommendations and goals developed. Form B - Professional Dialogue & Development Plan & Form E - Summative Evaluation Report
Second & Third -Year Probationary Teachers:
- The teacher will conduct a performance Self-assessment using the Professional Guide for Teaching prior to a professional dialogue conference. Form A - Professional Guide for Teaching
- Prior to October 1: Administrator and teacher will have a professional dialogue conference to identify strengths, areas for growth, and recommendations. - Form B - Professional Dialogue & Development Plan. Targets for goal development, and a timeline for continuous goal progress monitoring will be developed using Form C - Teacher Goals
- Prior to March 1: Administrator will conduct two (2) documented observations related to established goals for the year. These observations should be followed up with feedback from the supervisor within two school days after the observation. Form D - Documented Observation
- Prior to March 1: Administrator and teacher will have an evaluation summary conference at which time goal progress will be evaluated, strengths and weakness and recommendations and goals developed. Form B - Professional Dialogue & Development Plan & Form E - Summative Evaluation Report

PROCEDURES FOR NON-PROBATIONARY TEACHERS
All non-probationary staff members will receive, at a minimum, at least one (1) documented observation annually and one evaluation that results in a written summary report, every three (3)years.
Non-Probationary Teachers - Comprehensive Summative Year: Non-probationary teachers will have a Summative Evaluation Report every 3 years
- Teacher will conduct a performance Self-assessment using Form A - Professional Guide for Teaching prior to a professional dialogue conference.
- Prior to November 1: Administrator and teacher will have a professional dialogue conference to identify strengths, areas for growth, and recommendations. - Form B - Professional Dialogue & Development Plan. Targets for goal development, and a timeline for continuous goal progress monitoring will be developed using Form C - Teacher Goals
- The administrator will conduct a minimum of one (1) documented observation related to established goals during the year. These observations should be followed up with feedback from the supervisor within two school days after the observation.
- Prior to May 20th: Administrator and teacher will have an evaluation summary conference at which time goal progress will be evaluated, strengths and weaknesses and recommendations and goals developed. Form B - Professional Dialogue & Development Plan & Form E - Summative Evaluation Report - Form D - Documented Observation optional
Required Forms to be submitted to the district office as part of the summative evaluation :
- Form B - Professional Dialogue & Development Plan
Form E - Summative Evaluation Report
Non-Probationary Teachers - Goal-Centered Years: Non-probationary teachers on the years between their comprehensive summative evaluation year will engage in a more self-directed growth plan. This will be less formal and goal driven with at least one documented observation as per state law.
- Teacher will conduct a performance Self-assessment using Form A - Professional Guide for Teaching prior to a professional dialogue conference.
- Prior to November 1 Administrator and teacher will have a professional dialogue conference to establish areas for growth, targets for goal development, and a timeline for continuous goal progress monitoring. - Forms B & C
- The administrator will conduct a minimum of one (1) documented observation related to established goals during the year. These observations should be followed up with feedback from the supervisor within two school days after the observation.
- Prior to May 20th Have an evaluation summary conference at which time goal progress will be evaluated and future recommendations and goals developed. - Form E
