5230 School Admissions - Physicals and Immunizations
Physicals
School Nurse Practitioners should encourage parents of students needing health physicals and immunizations to purchase those services from physicians or public health.
If these services cannot reasonably be provided by out of school agencies, in the judgment of the School Nurse Practitioner, they may provide the services during school time as follows:
1. The fees are as follows:
a. Athletic physicals - $10.00 when offered by the physicians group prior to the start of school and $20.00 for those students who neglect to take advantage of the group program ($10.00 fee for new students to the District who could not take advantage of the group process.)
b. School admission physicals - $20.00
2. Fees may be waived by the School Nurse practitioners, if, in their judgment the student/parent cannot afford to pay or for any other good reason.
3. All money received through health services will be deposited in the activity fund of each building during the school year and returned to the District general fund as revenue at the end of each school year. None of this money could be expended for any purpose out of the building activity fund.
Immunizations
Immunization requirements will be strictly enforced for all students. Students who do not meet the requirements of law will be denied attendance according to C.R.S. 25-4-902. Students must submit a Certificate of Immunization or Exemption to attend school.

* Measles, mumps and rubella vaccines must have been administered on or after the first birthday to be acceptable for certification. Beginning July 1, 1992, 7th graders must have had two measles doses, two mumps doses and two rubella doses; if the student received a second measles dose prior to July 1, 1992, the second rubella and mumps doses are not required. The measles, mumps and rubella doses must have been administered on or after the first birthday and at least one month apart. Buy July 1, 1997, all students in grades 7-12 must comply.
** One hib vaccine dose must have been administered at age 12 months or older. Children who are 5 and older are exempt from the Hib requirement.
*** Beginning July 1, 1997, all day care students age 18 months through 4 years of age, kindergarten and 7th graders must have had three doses of hepatitis B vaccine.
*** Beginning July 1, 1997, all students less than 18 months of age must meet the minimum number of doses required for their age.
*** By July 1, 1998, all students in grades K-1 and 7-8 must comply. By July 1, 1999, all students in grades K-2 and 7-9 must comply. By July 1, 2000, all students in grades K-3 and 7-10 must comply. By July 1, 2001, all students in grades K-4 and 7-11 must comply. By July 1, 2002, all students in grades K-5 and 7-12 must comply. By July 1, 200e, all students in grades K-12 must comply.
The students doctor or clinic may recommend additional doses.
To comply with state law one of the following must be provided to the students school:
1. A completed Certificate of Immunization from a licensed physician or authorized representative of the department of health or local health department certifying that the student has received minimum immunizations as indicated above.
2. If a students certificate of Immunization is not up to date, the parent, guardian or emancipated student has 14 days to provide documentation that the next required immunization was given and submit a written plan for completion of all required immunizations. If the plan is not fulfilled the student shall be expelled or suspended from school for non-compliance as noted in C.R.S. 25-4-907.
3. After receiving direct personal notification that the students Certificate of Immunization is not up-to-date according to the requirements of the state board of health, the parent, guardian or emancipated students shall submit a written authorization for the first set and all subsequent vaccinations requesting that the local health officials administer immunizations within 14 calendar days.
4. Statement of Exemption to Immunization Law printed on the reverse side of the Colorado Department of Public Health and Environment Certificate of Immunization:
a. Medical exemption signed by licensed physician stating that the students physical condition is such that immunizations would endanger life or health or is otherwise medically contraindicated; or
b. Religious exemption signed by parent or guardian or emancipated student that (s)he is an adherent to a religious belief opposed to immunizations; or
c. Personal exemption signed by parent or guardian or emancipated student that (s)he is an adherent to a personal belief opposed to immunizations.
