5311 Parent Permission and Release Form for Photographs/Information


PRINT AND ELECTRONIC MEDIA: Occasionally the school district, the local, regional, or state media publish student work and pictures related to curriculum and/or a wide range of activities. The school district requests permission from parents/guardians to use and/or release pictures and information related to students to the media for use in newsletters, newspaper, news stations, etc. (Examples include but are not limited to: drama productions; the school yearbook; honor roll or other recognition lists; graduation programs; and sports activity sheets showing weight and height of team members, vocational or arts awards, etc.).

This release does not pertain to the internet.
